I don’t understand why one phrase uses na (em+a) but the other is just o.

Olá, @lorraineg2011. Thanks for asking. In speech, it’s common to say o instead of no in cases like this, but it’s more correct to keep the preposition there. So, ideally, it should be Reformámo-nos no ano passado. The exercise will be amended - sorry about that.
